Names and addresses of all employer(s) for the past two years

Residence addresses for the past two years

  (if renting, please include all Landlord names, addresses, and phone numbers)

Address, mortgage, tax, and insurance info. for all other real estate owned

Name and phone number of homeowner’s insurance agent

  or a copy of homeowner’s insurance Declaration Page

Homeowner Association contact information (condos, PUDs, and townhomes)

Divorce Decree or Legal Separation (if applicable)

Bankruptcy Discharge (if applicable)

q Gift Letter (for gifts of equity or gift funds)

HUD-1 Settlement Statement on property recently sold 

Certificate of Eligibility or DD214 (for VA loans only)

12-months or 24-months bank statements (for qualifying alternative doc. programs)

Real Estate Agent contact information or Purchase Agreement (for all new purchases)
